At Ember Vale, we believe that supported living should help people build confidence, achieve personal goals and make meaningful progress — on their terms.

We support adults aged 18+ with individual needs to grow skills, increase independence and thrive in ways that matter most to them. Our approach to moving on and measuring outcomes reflects this focus — it’s purposeful, individualised and strengths-based.

What “Moving On” Means at Ember Vale

“Moving on” doesn’t necessarily mean leaving support entirely — it means:

- Gaining new life skills

- Feeling more confident in daily living

- Increasing independence in practical areas

- Reducing reliance on structured support where appropriate

- Exploring next steps in education, work, relationships or community life

Progress is personal, not universal. What is meaningful for one person may look very different for another — and that’s okay.

How We Measure Outcomes

We use outcomes that reflect what matters most to the individual, rather than generic checklists. Outcomes are co-created with each person and reviewed regularly. They are:

Supporting Transitions

When a resident is ready to progress to the next stage of their journey, we support them in a planned and positive way.

This might include:

- Creating a practical transition plan

- Linking with future support networks

- Preparing for a change in living environment

- Sharing information with consent and dignity

Our aim is to make transitions smooth, empowering and confidence-building — not rushed or overwhelming.
