At Ember Vale, we work with professionals, families and commissioning teams to provide safe, tailored supported living placements for adults aged 18+ with individual needs.

We understand that finding the right placement is about more than a room — it’s about compatibility, wellbeing, skills development, long-term progress and person-centred support.

Our referral and placement process is structured, transparent and evidence-based — designed to match individuals with the right environment and support package that reflects their strengths, aspirations and needs.

Who We Support

We accept referrals for adults aged 18 years and over with individual needs who would benefit from structured supported living that:

- Builds independence and life skills

- Encourages community participation

- Offers respectful, skilled support

- Promotes wellbeing and confidence

This includes individuals with, though not limited to:

- Autism spectrum conditions

- Learning disabilities

- Emotional wellbeing needs

- Complex support needs

- Transitional support needs from other living arrangements

We can discuss bespoke support options on a case-by-case basis.

Our Placement Process

We follow a structured, best-practice process which includes:

1. Initial Enquiry

We gather basic information about the individual’s needs, preferences and current circumstances.

2. Pre-Referral Discussion

A conversation between the referrer and our team to clarify support requirements, suitability and

compatibility.

3. Assessment & Matching

We complete an in-depth assessment to determine whether Ember Vale is the right fit, and identify the most suitable home and support resources.

4. Placement Planning

A personalised support and transition plan is developed in collaboration with the individual, referrer, family and professionals.

5. Moving In

We support the transition into the home, providing structured orientation and settling-in support to ease adjustment.

Feedback, Review & Ongoing Support

Placements at Ember Vale are not static — progress is regularly reviewed with residents, families and professionals to ensure outcomes remain meaningful and achievable. We measure progress against agreed goals, regularly update support plans and adjust support intensity as needed.

Quality, Safeguarding & Compliance

Ember Vale is committed to safe and accountable practice. Our referrals and placements reflect values embedded in supported living best practice.

We are actively strengthening our governance frameworks and aligning our systems with recognised standards expected by the Care Quality Commission (CQC), including:

- Safeguarding and risk management

- Staff competency and training

- Policies and procedures aligned with regulatory expectations

- Transparent quality assurance reviews
